Your Medicare card and health fund details

Your referral (required for a Medicare rebate)

Please email a copy to our rooms if you are able to do so

All current distance and near vision glasses, contact lenses and eye drops

X-rays, blood test results or other documents

Please bring any relevant results of investigations or medical correspondence relating to your eye condition.

Sunglasses and arrange for someone to drive you home.

As your eyes will be dilated, you CANNOT drive for at least 3 hours after dilation drop administration.

Please arrive at least 15 minutes prior to your appointment time to allow time to complete registrations forms.

Translation services are available.
If you need a translator please allow our reception staff one weeks notice.

Unlike GP clinics, ophthalmology clinics operate in a different manner due to their speciality.

  1. Upon arrival, a copy of our New Patient Registration Form, Privacy Consent and Medical History forms will need to be completed.
  2. You will see our Clinical Assistant to go through some initial examinations and then return to the waiting room (for all patients 4yrs and above).
  3. The Ophthalmologist will then call you through for the examination – depending on your diagnosis/complaint the Ophthalmologist will instil dilating eyedrops (approximately 15+ minutes to dilate) returning you back to the waiting room.
  4. Once your pupils are dilated, the Ophthalmologist will examine your eyes further – depending on your diagnosis further examinations/scans may need to be completed (the tests may attract further fees however, this is not known until the day).

After your examination, you will need assistance to return home as you cannot drive for approximately 3 hours. It is advisable to make arrangements for someone to drive you home.
